What are DOT physicals?

A Department of Transportation (DOT) physical is a special exam intended to ensure that people who drive commercial vehicles are healthy enough to withstand the rigors of those jobs. They are required when an individual begins working in commercial transportation, and periodically once those careers begin.

Unlike many specialized physicals, which usually focus on making sure an individual is healthy enough to avoid harm when engaging in certain activities, a DOT physical is intended to protect not just the drivers, but the public with whom they share the roadways. 

DOT physicals are required for drivers who operate a vehicle designed to carry more than 15 people (or eight if the driver is paid) and those who drive vehicles that weigh more than 1,000 pounds or carry hazardous materials.

What should I expect during DOT physicals?

Many elements of a DOT physical are similar to what you’d expect from a routine health exam with your primary care provider. You begin with a discussion of your personal health history, including your mental health and any history of substance abuse.

Be open and honest with your provider about any medications or supplements you’re taking, as well as any conditions you’ve recently been treated for. 

​

Next comes a physical exam. Expect brief checks of the following:

  • Vision and hearing

  • Heart function

  • Lung function

  • Skin condition

  • Overall appearance

  • Height and weight

  • Blood pressure

  • Extremities and joint function 

  • Circulatory system

A urinalysis is also part of the process. This simple lab test checks for a range of other health issues, and your practitioner detects an area of concern, they advise you on necessary additional screenings.

What are some other kinds of specialized physicals?

Many people need to come in for one or more specialized physical exams over the years. These visits are similar to your annual physicals, but they may slightly vary in terms depending on your organization.

If you’re preparing for a surgical procedure, your surgical team might require you to come in for a preoperative physical exam. These assessments ensure you’re healthy enough for surgery and the recovery process. These visits might have very specific health markers that must be met.

Kids need specialized physicals for school enrollment. If they want to play organized sports, a sports physical is usually part of the process. Even some camps and youth organizations require physicals prior to allowing kids to enroll or participate.
